Debtors
When you filed your bankruptcy petition, the court issued you a court date known as the Meeting of Creditors (or 341 meeting). This meeting must be attended and is your testimony under oath. Failing to attend will result in a dismissal of your case. These meetings are held at the Foley Federal Building, located at 300 Las Vegas Blvd. South, Las Vegas, NV 89101, Room 1500. Please make sure to bring the following documents to your scheduled meeting of creditors.
· Picture ID issued from a governmental agency. This includes: driver’s license, passport, government issued identification card, or legal Residents Alien Card.
· Social Security number verification from a governmental agency or 3rd party. This includes Social Security card, payroll stub, W-2, letter from the Internal Revenue Service. *Please note that tax returns are self-prepared and therefore are not valid verification.
To avoid unnecessary continuances and/or a dismissal of your case, the following documents must be submitted at least ten (10) days prior to your meeting of creditors (the 341 meeting):
· Completed copies of your Bankruptcy Petition, Schedules, Statement of Financial Affairs, Counseling Certificates, and any Amendments. These documents should be filed with the Bankruptcy Court located on the 4th floor of the Foley Federal Building. The Trustee does not file any documents on behalf of debtors.
· Completed Bankruptcy Questionnaire
· Statements from all financial accounts for the 4 months prior to filing the bankruptcy petition and statements which show the balance of all accounts on the date which you filed the bankruptcy petition.
· Verification of income for the 4 months prior to filing the bankruptcy petition and statements which include the date which you filed the bankruptcy petition. This verification can come from paystubs, unemployment records, etc.
· Copies of your completed tax returns for the current and preceding tax year.
If you are represented by an attorney, these documents should be provided by your attorney through the DockLink system. For more information regarding this system, please refer to the DocLink website.
If you are a Pro Se Debtors (those filing without an attorney), please read the information below carefully and refer to Bankruptcy Resources page.
The Trustee’s Office does NOT accept hand delivered documents. Any and all information provided will not be returned. Please make sure to send copies—not originals. You may submit your information in one of the following ways:

In addition to the documents requested above, you will need to submit the following additional documents:>
· Completed Statement of Debtor not Represented by an Attorney
· Completed Statement of Social Security Number(s)
· Copy of any property settlement, divorce decree within 4 years of filing for bankruptcy.
· Copies of any financial documents given to any entity within 2 years of filing for bankruptcy.
